A best practice for shared decision-making is to use a decision aid
with patients during the counseling interaction. The contraception
decision aid “Contraception (Birth Control) Guide” is designed to
support the exploration of patients’ values and preferences and
center patients. To accompany the decision aid, a set of 17 method
information sheets were developed to support patient education.
There is also a provider guide and patient guide to learn about the

Extended Use of LARC Methods 4/2/20
There is research data that supports extended use for most
methods of long-acting reversible contraception (LARC) available
in the US. Studies have shown that some hormonal IUDs (Mirena,
Liletta), as well as the copper IUD (Paragard), and the
contraceptive implant (Nexplanon) are effective beyond their
FDA-approved duration. This document outlines those research
findings.

Social Determinants of Health
(SDOH) Editable Question Set 11/08/22
Here you will find a compilation of evidence-based SDOH
screening questions that have been tested in clinical and/or
research settings. These are being provided to you as a starting
point for use in your own clinic. We are mindful of the fact that a
standard visit is 15 minutes and there are a lot of questions to ask.
You may adapt them as you see fit, including editing language, and
the order in which they are asked.

(SDOH) Launch Email Template 11/08/22
Here you will find a drafted email for you to send out to your team
about the implementation of social determinants of health (SDOH)
screening protocol at your practice.
Social Determinants of Health
(SDOH) Referrals 11/08/22
In some circumstances, the information obtained from a social
determinants of health (SDOH) assessment tool not only improves
the immediate health care provided, but also creates an
opportunity for a patient to be referred for other services unrelated
to the visit that will impact other areas of the patient’s health. Once
an assessment identifies a social need, clinicians or health care
providers must refer patients to organizations for additional
resources and services. To ensure referrals to high-quality external
resources, health care providers should consult trusted
clearinghouses of quality providers such as those listed in this
document.
Social Determinants of Health
(SDOH) Toolkit 11/08/22
This toolkit provides an in-depth explanation of social
determinants of health (SDOH), their impact on sexual and
reproductive health as well as overall health, and the importance
of integrating SDOH screening into clinical care. We provide
guidance and recommendations to help you develop your clinic’s
workflow for conducting SDOH screening.
Social Determinants of Health
(SDOH) Trainings 11/08/22
Part of the importance of pre-determining and standardizing your
clinic’s workflow around conducting SDOH screening is that asking
these questions without proper training and infrastructure to react
and refer patients appropriately can cause more harm than good.
Many of these questions are sensitive and disclosure may be
challenging. Therefore, staff may need training to ensure the
successful implementation of a new SDOH screening protocol at
your practice. Here you will find a list of trainings on different
dimensions of SDOH.
Social Determinants of Health
(SDOH) Worksheets 11/08/22
These worksheets will help you to plan your clinic’s workflow for
asking each social determinants of health (SDOH) screening
question. Completing this worksheet will ensure that you have
thought through all the necessary components of developing an
SDOH screening plan.
SDOH Short Form
08/15/23
This tool was designed to assist sexual and reproductive health
(SRH) agencies in Massachusetts in meeting the SRHP standards
for social determinants of health (SDOH) screening. While this tool
is not the only way to meet the standards, it can be time- and
resource-intensive to design a comprehensive SDOH screening
tool. Thus, this tool was developed as an option for clinics to
implement in part or in whole, depending on their needs and
existing screening infrastructure. Providers and practices outside
of Massachusetts are welcome to use and adapt this tool.

note, while this tool was designed using an evidence-informed
process, it has not yet been validated.
In this set of resources you will find the survey, scorer, and
implementation guide. Before using this tool with patients, it is
critical that your clinic has established referral pathways for all
SDOHs for which you will be screening. We suggest that you
review the implementation guide for tips for developing
appropriate referrals. We have provided PDF and editable versions
of the survey and scorer so you can adapt the referrals and
community resources as needed.
